Prior to Your First Appointment
Please contact your insurance company’s member services or employer’s human resources department to ensure that Center for Collaborative Health is in network and to understand your deductible/co-pay responsibilities (two standard service codes to check are 90791 and 90837).
If Center for Collaborative Health is not in network, please contact us to confirm and review your options for payment.
Please complete all forms sent to your email via our client portal through Therapy Notes.
Please take the time to read the following policies and procedures. We request that you save the documents to your personal computer rather than printing them. Copies can be sent to your email upon request. These forms will also be available for you to review in the office.
Electronic Health Record (EHR)
To provide our clients with the most efficient care, the Center for Collaborative Health has implemented an Electronic Health Record (EHR). We have chosen a HIPPA-compliant platform that meets requirements for all Minnesota state laws.
The Client Portal is a secure account that will allow you to:
- Complete and submit Intake paperwork prior to your first appointment
- Update our contact and billing information
- Additional services will be added, including viewing and paying your account balance
To set up your Client Portal account, upon scheduling your first session at CCH, your provider will send you a “Welcome Email” using the email and birthdate you provided. The “Welcome Email” will provide you with instructions on how to set up your portal account as well as completing the intake paperwork for your first appointment.
You will receive an email alerting you to new documents in your portal account anytime they are requested by your provider.
What to Bring to Your First Appointment
- Your insurance card (if applicable)
- List of any medications you are currently taking and information on your current medical provider
If you are coming for a psychological evaluation, we request that you email us the following prior to your session:
- Any previous evaluations completed (including IEP or 504 Plan)
- Any police or court documents regarding your case
- Contact information for any individuals involved in your case including CPS worker, Probation Officer, and/or lawyer.
If you are unable to send us these documents by secure email, please bring the documents with you and we will scan the documents in the office. No need to bring us personal copies.
